NEW / RENEWAL TRICYCLE FRANCHISE


 

 

1. Certificate Of Membership signed by the TODA/Association President, PMTR President or PMTR Duty Officer

2. Official Receipt (OR) from Treasurer's Office as payment for Tricycle Franchise

3. Certificate from Engineering Office

4. Copy of OR/CR from LTO

5. Copy of Authority for Tricycle Service from Office of the Mayor

If the requirements above are completed, the Office of the Sangguniang Bayan will issue an Authority to Operate Motorized Tricycle for Hire which is one of the requirements needed in LTO.

 

NEW / RENEWAL TRICYCLE MUNICIPAL PLATE & STICKER


 

 

1. Duly Accomplished Clearance from Office Of The PMTR signed by Association President, PMTR Secretary, and PMTR President

2. Latest copy of OR and CR from LTO (New Registered)

3. Copy of receipt from Treasurer's Office as payment for sticker and municipal plate.

If the three (3) requirement are completed, the Sangguniang Bayan will then issue the Sticker & Municipal Plate.

 

APPLICATION FOR ACCREDITATION


 

 

1. Duly accomplished Application Form for Accreditation

2. Board Resolution

3. Certificate of Registration

4. List of Current Officers and Members

5. CY Annual Accomplishment Report

6. CY Financial Statement

7. Profile indicating the purposes and objectives of the organization

8. Copy of the Minutes of the CY_Meeting of the organization

1. Upon receipt of duly filled up Application Form of Accreditation and required documents the item is subject for the First Reading and referral to Committee on Accreditation and for schedule in an En Banc Committee Meeting and Hearing


2. Committee Report subject for approval to the Second Reading and Third & Final Reading


3. Approved for signature of the SB Member


4. Issuance of Certificate of Accreditation
