ISSUANCE OF CIVIL REGISTRY DOCUMENTS
Civil Registry Documents are documents issued to clients as Certified Photocopy of Birth, Marriage and Death Certificate, Court Decrees and Legal Instruments such as Affidavit of Paternity/Acknowledgement, AUSF RA 9255 (Affidavit to Use the Surname of the Father), Legitimation and Amended Civil Registry Documents affected by Correction of Clerical Error thru RA 10172 and RA 9048, Court Order/Decree such as Correction of Entry, Annulment, Adoption, etc. affecting the civil status of the person in the appropriate registers as mandated by Act No. 3753, the Civil Registry Law.
Who may avail of the service?
● Citizens of the Republic of the Philippines and other interested parties
Authorization and ID of the document owner together with the requesting party's ID if he/she is not ANY of the following:
● Owner of the document
● His/her parent
● His/her spouse
● His/her legal guardian

Registration Of Vital Events (BIRTH, MARRIAGE, DEATH, others)
Vital events occurring in the life of an individual such as birth, marriage and death as well as all court decrees affecting the civil status of an individual should be registered/reported.
Who may avail of the service?
● Citizens of the Republic of the Philippines and other interested parties
For Timely Birth Registration:
• If legitimate child, marriage contract of parents
• If illegitimate child and to be acknowledged by the father, the presence of the father at the LCRO is required to sign on the COLB with his valid ID and with the conformity of the mother
For Late Registration:
• PSA negative certification of birth
• Affidavit of Parents and two disinterested persons for late registration of birth
• Certificate of Residency from the Office of the Barangay Captain with certification stating the facts of birth with name of child, date of birth, place of birth and name of parents
• Documents stating the facts of birth
Ø Baptismal Certificate
Ø Marriage contract if married
Ø Voter’s application for registration
Ø Philhealth Member’s Data Record / SSS Records / Pag-IBIG records
Ø School Records
Ø And other documents stating the facts of birth
• National ID of the registrant
• Any two documentary evidence showing the identity of the parents such as but not limited to his/her COLB, government issued ID, marriage certificate of the parents if applicable of death certificate for the deceased parents
• Unedited front-facing photo of the registrant to be attached to the application, 2X2 size with white background
If the solemnizing officer is from other religious affiliation, the photocopy of the Certificate of Authority to Solemnize Marriage from PSA is to be presented.
For Late Registration:
• PSA negative certification of marriage
• Copy of the marriage contract to be registered
• CENOMAR of groom and bride
• Affidavit for late registration of marriage of the registrant and two disinterested persons
• And any other documents needed to submit after the evaluation and verification of the LCRO personnel
Informant should be the legal spouse if married, or the nearest relative
Note: Death Certificate will be registered after reviewed and signed by the Municipal Health Officer
For Late Registration:
• PSA negative certification of death
• Affidavit for late registration of the nearest relative of the deceased and two disinterested persons
• Certified true copy of the certificate of burial, cremation or other means of corpse disposal, burial expenses receipts, burial permit, cremation permit, etc.
1. AFFIDAVIT OF PATERNITY/ACKNOWLEDGEMENT
- Unannotated Birth Certificate
- Notarized Affidavit of Paternity/ Acknowledgement
- Valid IDs of parents
- With the conformity of mother
2. AUSF RA 9255 (Affidavit to Use the Surname of the Father)
- Unannotated Birth Certificate
- If acknowledged by the father, notarized Affidavit to Use the Surname of the Father executed by the mother
- If the child aged 7- 17 years old, the child will execute the AUSF with attestation of the mother
- For a child who is of age (18 years old and above), he himself will execute the AUSF
- If not acknowledged by the father, notarized Affidavit of Paternity and Affidavit to Use the Surname of the Father
- All affiants will submit valid IDs
3. LEGITIMATION
- Unannotated Birth Certificate
- Notarized Affidavit of Legitimation
- Affidavit of Minority of parents, if parents’ age is 18 years old and below
- Certified Photocopy of Marriage Contract of Parents, Valid IDs
- CENOMAR from PSA
4. Amended Civil Registry Documents affected by Court Order/ Decrees such as Correction of Entry, Annulment, Adoption, etc.
- Certified Photocopy of Unannotated civil registry document
- Certified Photocopy of the Finality of Court Decision / Court Order
- Certification of Registration of the court decree/order from the City/Municipal Civil Registrar’s Office where the court is functioning
Note: Amended Civil Registry documents affected by Court Order should be issued after ten (10) days upon receipt of completed documents

ISSUANCE OF MARRIAGE LICENSE
A Marriage License is a document issued to contracting parties after ten days posting of the Application for Marriage License
Who may avail of the service?
● Citizens of the Republic of the Philippines and other interested parties
1. Birth Certificate of the contracting parties (with 2 xerox copies)
2. Valid IDs (with 2 xerox copies)
3. 2 pcs. 2X2 ID picture each
4. CENOMAR from PSA with Official Receipt (with 2 xerox copies)
5. Parental Consent/Advice if the contracting parties is under 25 years old / valid IDs of parents
6. Certificate of Marriage Counselling and Family Planning from Municipal Social Welfare and Development Section
7. When either or both of the contracting parties are citizens of a foreign country, legal capacity to contract marriage issued by their representative diplomatic or consular officials should be presented and copy of passport with date of arrival. (with 2 xerox copies)

CORRECTION OF CLERICAL ERROR (CCE) UNDER RA 9048
Petitions for correction of clerical error to correct the typographical error in an entry in the civil register without need of a judicial order.
Who may avail of the service?
● Document’s owner / spouse / children / parents / siblings / grandparents / or any other person authorized by law or by the owner of the document and the authorization shall be in the form of Special Power of Attorney
• PSA and LCRO copy of the document to be corrected
• At least three public or private documents showing the correct entry/entries upon which the correction or change shall be based:
Examples of these documents are the following:
Ø Baptismal Certificate
Ø Voter’s Certification / Voter’s application for registration
Ø Employment record
Ø GSIS / SSS records
Ø School record
Ø Medical record
Ø IDs / NBI / Police Clearance
NOTE: The MCR may also require other documents in addition to what had already been submitted by the petitioner
Examples of these documents are the documents of the parents / siblings / offspring such as Birth Certificate / Baptismal Certificate / Marriage Contract / Death Certificate
