APPLICATION OF COMMUNITY-BASED DRUG REHABILITATION PROGRAM (CBDRP) - COURT MANDATED

 

 

 

 

Who may avail of the service?

 

● Applicant-Client who are registered voter and permanent resident in Teresa, Rizal


 

 


● Court Order

● Attach Drug Dependency Examination (DDE), if available


These documents can be secured at the Regional Trial Court having jurisdiction of the case

1. Proceed to Municipal Anti-Drug Abuse Office (MADAO) and submit the court order. Provide the application form upon client request. Five (5) Minutes

2. Fill-out and submit the completed form, documents and attchments. Receive and verify the form and documents. Ten (10) Minutes

3. Wait for the interview or assessment with the rehabilitation team. Conduct the interview or assessment with the client. Twenty (20) Minutes

4. If approve, applicant-client will be enrolled to CBDRP. Enroll the client and provide information about the program schedule. Five (5) MInutes

5. Attend sessions based on the program schedule and follow the rehabilitation plan. Monitor client participation and progress in rehabilitation sessions. Six (6) Months or Twenty-Four (24) Weeks or may be extended

6. Finish the program and receive a certificate of completion. Issue the certificate of completion upon successful program completion. Ten (10) Minutes

1. Proceed to Municipal Anti-Drug Abuse Office (MADAO) and submit letter from the barangay Provide the application form upon client request. Five (5) Minutes
